Enhanced Care Management (ECM) enables us to provide comprehensive, whole-person care to those who need it most, with a special focus on the justice-involved population, homelessness, substance use disorders, and serious mental illness. Leveraging our strong community connections and hands-on approach, we offer intensive case management for our participants at no cost. Each participant is paired with a Lead Care Manager (LCM) who oversees their personalized care plan, tailored to help them achieve their goals. We support them every step of the way, ensuring they successfully complete each milestone.

With years of experience, we are well-equipped to make a significant impact. Our ultimate goal is to increase employment and reduce recidivism of incarceration, homelessness, relapses from substance use, and emergency room hospital stays in Kern County.
